Claims & Appeals

The claim, with its whole story.

When a member calls about a claim, the answer they hear and the evidence supporting it usually live in different systems—and the interaction record is assembled later, from memory. This integrated reference scenario treats them as one object: the claim, its adjudication timeline, the case it opened, and the attributable interaction record.

Audit trail

Voice inquiry joined to claim record · reference audit #4118. The guided claims demo uses the same synthetic member and claim context to show how a voice interaction, claim, and audit event share one governed record.

From call to claim to case to audit — one platform.

Run the voice demo as Maria to see the interaction side of this reference scenario, then inspect CLM-58821 here to see the corresponding claim and evidence model. The interaction, claim, case, and evidence remain connected in one governed record.

There is no seam between those four cells, and that is the whole claim. Four systems exchanging files would need three — a reconciliation between each pair, and a version of the truth that is only correct until the next batch runs.

When someone disagrees

Two ways every decision gets questioned.

An appeal is a determination someone disagreed with. A member call is a determination being explained. The same spine carries both. California SB-1120 prohibits AI tools from making medical-necessity determinations; a competent licensed physician or health care professional must make the determination. Since January 2026, CMS-0057-F has required impacted payers to give a specific reason for denied non-drug prior-authorization requests; Prior Authorization APIs generally follow in 2027. The record is what survives either one.
